What is an Apicoectomy?

An apicoectomy, also known as root-end surgery, is a minor surgical procedure performed to remove persistent infection or inflammation from the tip (apex) of a tooth root when conventional root canal treatment or re-treatment has not adequately resolved the problem. The procedure aims to eliminate the affected tissue while preserving the natural tooth whenever possible.

When is an Apicoectomy Recommended?

An apicoectomy may be considered when:

• Infection persists around the root tip after root canal treatment
• Re-root canal treatment is not feasible or is unlikely to be successful
• A persistent lesion or inflammation is present around the root apex
• A root canal cannot be adequately accessed or retreated through the crown
• A small fracture or other localized problem is present near the root tip

A clinical examination and appropriate dental imaging are usually required before treatment.

How is an Apicoectomy Performed?

The procedure generally involves:

• Local anesthesia to numb the treatment area.
• A small incision in the gum to access the affected area.
• Removal of the infected or inflamed tissue around the root tip.
• Removal of a small portion of the root tip when indicated.
• Cleaning and sealing of the root end.
• Suturing the gum to allow proper healing.

Is an Apicoectomy Painful?

The procedure is performed under local anesthesia, so significant pain during treatment is generally avoided. Mild swelling, tenderness, or discomfort may occur after the procedure and usually improves as healing progresses.

Can an Apicoectomy Save My Tooth?

In appropriately selected cases, an apicoectomy can help eliminate persistent infection and preserve a natural tooth that might otherwise require extraction. The prognosis depends on factors such as the condition of the tooth, surrounding bone and tissues, quality of the previous root canal treatment, and the presence of any cracks or fractures.

What Should I Expect After an Apicoectomy?

Follow your dentist’s postoperative instructions carefully. You may be advised to maintain gentle oral hygiene around the surgical area, avoid disturbing the site, and attend a follow-up appointment for evaluation and suture removal when required. Persistent pain, increasing swelling, fever, or unusual discharge should be reported to your dentist promptly.
